ACO REACH + LEAD
CMS's optional Substance Access Beneficiary Engagement Incentive allows approved ACO REACH participants to consult with eligible beneficiaries about eligible hemp products and furnish those products under defined requirements and safeguards.
Eligible ACO REACH participants may begin offering the BEI on April 1, 2026. It becomes available to eligible LEAD participants on January 1, 2027.

Product offering
Phytava products are full-spectrum, plant-based, and non-isolated, preserving the naturally occurring cannabinoid and terpene profile of the hemp extract.
The current capsule lot was independently analyzed by MCR Labs Massachusetts. The report documents cannabinoid potency and terpene composition and includes safety screens for heavy metals, microbiological contaminants, mycotoxins, and pesticides.
Capsules
A single-source, single-strain, organically grown hemp-derived CBD capsule infused into organic cold-pressed hemp heart oil.

Topical
Phytava’s full-spectrum topical cream is a 2 fl oz external-use formula with 500 mg of CBD, combining whole-plant hemp extract with a soothing botanical base for targeted daily support.
The formula consists of shea butter and herbal tea emulsion with hemp seed oil, subcritical CO2 hemp extract, wintergreen, comfrey, wild lettuce, mint, menthol essential oil, and tea tree essential oil.

Co-Founder & President
Michael leads Phytava's strategy, partnerships, and product vision. A UMass Amherst graduate in soil management, he brings firsthand experience cultivating hemp in Massachusetts and a plant-first approach to the company's work. He focuses on organic practices, product development, quality documentation, and building practical partnerships with healthcare providers and community organizations in Massachusetts and beyond.
Co-Founder & Operations
Brian leads Phytava's operations and the path from farm to finished product. A graduate of the Isenberg School of Management at UMass Amherst, he brings a business and systems perspective to production, supply coordination, and partner support. He focuses on consistent execution, clear documentation, product quality, and repeatable processes for healthcare and community programs.
Physician Advisor & Clinical Champion
Neil Kudler, MD is a Massachusetts general internist with more than 30 years of experience caring for adults, including HIV care, substance use disorders, and palliative care. He has evaluated patients for medical cannabis since 2015. As Phytava's physician advisor and clinical champion, he guides education, medication review, follow-up planning, safeguards, and healthcare partnerships.
